About the Role:

As a Psychiatric-Mental Health Nurse Practitioner (PMHNP) at Hazelbrook, you’ll play a critical role in assessing, diagnosing, and treating individuals with serious mental health and substance use disorders.

  • Medication Management: Support treatment plan updates related to medication, monitor Medication Administration Records (MARs), provide a weekly medication group, and be available for impromptu phone calls regarding medication questions.
  • Group Facilitation: Conduct groups as needed to support clients with medication management and skill development
  • Individual Support: Hold weekly meetings with all clients individually to address their specific needs and progress
  • Documentation and Support: Complete documentation within 24 hours and meet key performance indicators (KPIs) on a monthly, quarterly, and yearly basis
  • Community Support: Provide periodic UA (urinalysis) sign-off and crisis support services at the Hazelbrook Community Center in partnership with the peer support team

Must Have:

  • Licensure: Active and unrestricted PMHNP license in the state of Colorado and Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC)
  • Collaboration: Ability to communicate and work effectively with the Clinical Director and Program Director regarding treatment plans
  • Availability: Availability to work a minimum of two times a week
  • Problem-Solving: Strong problem-solving skills and the ability to handle crisis situations effective
  • Cultural Competence: Sensitivity to and understanding of clients from diverse backgrounds, promoting an inclusive and respectful environment
  • Technology: Experience with Employee Health Records (EHR) systems, online medical records and ability to utilize translation technology.
  • Behavioral Health & Substance Abuse Experience: Prior experience working with individuals in behavioral health or substance abuse settings.

Nice to Have:

  • Group Facilitation Experience: Experience leading group sessions, particularly for substance abuse management.
  • Bilingual Communication Skills: Excellent verbal, written, and listening communication skills in Spanish and English

  • Location: At Behavioral Health home at 3658 Bahama St Aurora, CO 80013 and Hazelbrook Community Center at 14109 E Exposition Ave Aurora, CO 80012
  • Schedule:
  • Part-time availability required (minimum 2 days per week)
  • Flexible hours depending on client needs and availability
  • Full-time opportunities may be considered based on program growth and applicant interest

Compensation:

  • Hourly Rate: $70–$95/hour (depending on experience and credentialing)
  • Estimated Annual Range: $72K–$180K/year based on hours worked
